Payment and Disclosure

We will gladly prepare a written estimate if you desire (please ask the doctor or receptionist). ALL PROFESSIONAL FEES ARE DUE AT THE TIME SERVICES ARE RENDERED. We accept all major credit cards. There will be a service charge for any check returned unpaid. To prevent the spread of infectious diseases, all hospitalized patients must be current on all vaccines and free from intestinal and external parasites. The signature below authorizes this level of preventative care and the appropriate charges will be assessed in the discharge invoice. 

You are to use all responsible precautions against injury, escape, or death of my pet, but you will not be held liable or responsible in any manner in connection therewith as it is thoroughly understood that I assume all risks, even those including anesthetic. If the pet is not called for within 10 days after the time specified for return and if the doctor is not notified in writing of an alternate date within the 10 day period, the animal shall be considered abandoned and may be disposed of as the doctor sees fit. I understand that this does not relieve me from paying for all costs, including boarding cost. I’ve read and understand this.
